統合失調症の定量評価を目的とし, 水分子の拡散異方性を求めるMR拡散テンソル画像法(MR-DTI)を用いて大脳白質線維追跡を行なった.
統合失調症患者群と, 年齢, 性別, 利き腕をマッチングさせた健常者群を対象とした.
関心領域は左右大脳半球の上縦束とし, 脳梁膨大前部から, 脳梁の後端から4割前までを追跡領域とした.
患者群と健常者群, またそれぞれにおいての左半球と右半球において線維追跡の開始部における面積・拡散異方性の強さFAの平均と標準偏差・見かけの拡散係数ADCの平均と標準偏差を比較した.
その結果, 追跡開始部の面積において, 患者・健常者群比較では右半球において健常者群が有意に大きく, 左右半球比較では健常者は右半球の方が有意に大きいが, 患者は左右差が無く, 面積を比較することで統合失調症の定量評価ができる可能性が示された. |

We analyzed the cerebral white matter by using magnetic resonance diffusion tensor imaging (MR-DTI) that measured diffusion anisotropy of water molecules.
The goal of this study was quantitative evaluation of schizophrenia.
Diffusion tensor images were acquired in patients with schizophrenia and normal comparison subjects, group-matched for age, sex, and handedness.
Fiber tracking was performed on the superior longitudinal fasciculus, from the anterior splenium of corpus callosum to the coronal plane 40 percent ahead from the back edge toward the anterior edge of the corpus callosum for the comparison between the patient and comparison groups.
We analysed and compared the cross-section area on the starting coronal plane and mean and the standard deviation of fractional anisotropy and apparent diffusion coefficient along fibers in the right and left hemispheres.
In the patient group, the cross-section areas in the right hemisphere were significantly larger (p$<$0.05, t-test) than those in the left hemisphere, whereas there was no significant difference in the comparison group.
Furthermore, in the right hemisphere, the cross-section areas in comparison group were significantly larger (p$<$0.05, t-test) than those in the patient group.
These results suggest that we may evaluate disruption in white matter integrity in schizophrenia quantitatively by comparing the cross-section area of the superior longitudinal fasciculus in the right and left hemispheres. |
