| (in English) | Aging rate in Japan has been rising. And aging rate in the survey of 2014 was 26.0%. Meanwhile, the Ministry of Health, Labour and Welfare shows the concept of a community based comprehensive care for the living of elderly. And the demand for home care will go to increase. Staff of multidisciplinary of multiple agencies are involved in a user, cooperation between the staff of the multidisciplinary are important in home care. Therefore, we have developed the community based comprehensive care system "Nagoyaka Smile Net" as a platform system to support the implementation of the community based comprehensive care including multidisciplinary cooperation, evaluated and consideration for it. | |
