Computer-aided diagnosis scheme for detection of intracranial aneurysms

(in English) Cerebrovascular diseases is the 3rd leading cause of death in Japan. Therefore, a screening system for early detection of asymptomatic brain diseases, which is called Brain Dock or Brain Check-up, is widely performed. In this screening system, unruptured aneurysms are often detected in magnetic resonance angiography (MRA). The detection of unruptured aneurysms in MRA images is important because aneurysm rupture is the main cause of subaracnoid hemorrhage. However, it is often difficult for radiologists to detect small aneurysms because of the overlap between the aneurysm and the adjacent vessels on maximum intensity projection (MIP). Therefore, we have developed a computer-aided diagnosis (CAD) scheme for the detection of unruptured aneurysms in MRA images in order to assist radiologists’ interpretation as a “second opinion.” Our CAD scheme consists of two components: (1) to provide the locations of aneurysm candidate on MIP image and/or volume rendering image, and (2) to provide a new type of MIP image, the SelMIP image, with interested vessel regions only by manually selecting a desired cerebral arteries from a list. The sensitivity of our CAD scheme for the detection of aneurysms was 93.8% with 1.54 false positive per patient. Our CAD scheme will be useful in assisting radiologists to detect aneurysms and reducing the interpretation time.
