PURPOSE
To investigate extra- (EMCL) and intramyocellular (IMCL) lipid changes in skeletal muscle by means of proton magnetic resonance spectroscopy (1H-MRS) in male patients with obstructive sleep apnea (OSA).
MATERIAL AND METHODS
Eighteen male patients with documented OSA and eighteen age- and BMI-matched male volunteers (age ranged 50.7 +/- 5.7 vs. 50.4 +/- 6.9 years; BMI ranged 25.9 +/- 2.9 vs. 25.7 +/- 2.7, respectively) were consecutively accrued in this study. 1H-MRS of the right adductor magnus muscle was performed on a 1.5T clinical MR scanner. EMCL fraction was defined as the ratio of the integration of EMCL methylene (-CH2) and EMCL methyl (-CH3) to creatine resonances. IMCL fraction was defined as the ratio of the integration of IMCL (-CH2) and IMCL (-CH3) to creatine resonances. The strength, expressed by maximum voluntary contractions (MVC), and endurance of the thigh muscles were measured by using an isokinetic dynamometer (Cybex 6000). Variables of both groups were compared using paired sample t test.
RESULTS
OSA patients had significantly lower value of IMCL fraction (10.3 vs. 14.1, P = 0.03) and EMCL fraction (14.1 vs. 24.7, P = 0.01) when compared with normal volunteers. OSA patients had significantly lower value of muscle strength (134.0 vs. 158.7, P < 0.001) and endurance (43.6% vs. 61.6%, P < 0.001) when compared with normal volunteers. No significant correlation between the IMCL or EMCL fraction and the muscle strength or endurance.
CONCLUSION
1H-MRS demonstrated both decreased IMCL and EMCL content in male patients with OSA. No significant correlation between muscle lipid measurement and its function. |
