A musculoskeletal model-based estimation of shoulder muscle forces for elevating the upper arm in case of rotator cuff tears

(in English) Rotator cuff tear is one of the typical muscle disorders in shoulder. This tear is believed to change the activity pattern in shoulder muscles. The actual change in muscle activity, however, is still unclear. Then, it is strongly required in clinic that the change in muscle activity due to rotator cuff tear be clarified. The purpose of this study is to estimate the muscle forces in shoulder with and without rotator cuff tear during the abduction of the upper arm by means of computer simulation. The shoulder model consists of three different models as follows: 1) bone models including humerus, scapula and clavicle; 2) muscle models including all outer and inner muscles; and 3) 6-degree-of-freedom movement model of the abduction. We prepared three different abnormal shoulder models with slight, severe and complete tears in supraspinatus. Simulation results suggested that compensatory muscle activity due to rotator cuff tear could occur primarily in infraspinatus, subscapularis and the anterior and intermediate portions of deltoid.
