Changes of Excitability on Sensorimotor Cortex by Closed-Loop Brain-Machine Interface

(in English) A possible mechanism of brain-machine interface (BMI) assisted stroke rehabilitation of upper limb can be described as a closed-loop model, i.e. a patient receives visual or somatosensory feedback of normal brain activities following motor imagery, resulting motor learning. We, for evaluating this hypothesis, recruited fifteen healthy adults randomly assigning to one of three conditions; the closed-loop BMI (motor imagery with positive visual feedback), the unclosed-loop BMI (motor imagery with uncorrelated visual feedback) and the open-loop BMI (motor imagery without feedback). They trained motor imagery with the same assigned BMI for five successive days. Electroencephalogram (EEG) and near-infrared spectroscopy (NIRS) were recorded simultaneously, and the excitability of the primary sensorimotor cortex was evaluated. The result showed that event-related desynchronization of EEG, reflecting changes in cortical excitability, was increased in the closed-loop BMI, but hemodynamic response of NIRS was not change in all conditions (p<0.05). A closed-loop system of BMI rehabilitation is thus considered better in practice, and is expected to be capable of inducing stronger effects than other rehabilitation methods with motor imagery.
