An MR Diffusion Tensor Fiber Tracking Study on the Disruption of Cerebral White Matter Integrity in Alzheimer's Disease

(in English) The number of people with symptoms with dementia is now rapidly increasing in Japan, and the number of elderly people thought to be increasing more and more in the future. Therefore, evaluating dementia subjectively is necessary for early detection and treatment. In this paper, we analyzed Alzheimer's disease, which is one major form of dementia, by using tractography in cerebral white matter. Magnetic resonance diffusion tensor images (MR-DTIs) were acquired in 17 patients with mild Alzheimer’s disease, 17 patients with moderate Alzheimer’s disease, and 17 age- and sex-matched healthy comparison subjects. We selected regions of interests in the posterior thalamic radiation (PTR), anterior and superior corona radiata (ACR and SCR), cingulum (CG), and superior longitudinal fasciculus (SLF) and used the tractography to compare the number of fibers tracked and mean and standard deviation of fractional anisotropy, mean diffusivity, and curvature of fibers. As the result of the comparison, we found that there were significant and marginally significant differences among all groups and between two hemispheres so well. In particular, we found that the mean MD of left PTR, mean FA of right SCR, and mean curvature of SLF were influenced by the progression of Alzheimer’s disease. These results suggest the possibility of subjective evaluation of Alzheimer's disease.
