(in English) Safety is a primary factor for users. Not only emergency measures and recurrence prevention measures but also prevention measures based on the prediction should be taken for preventing any significant accident involving quality, reliability, and safety matter. This paper proposes prevention measures by using Top Event Mode, the result of the sequence of serious accidents occurrence analysis. It is important to capture Top Event Mode for every field. The procedure using Top Event Mode and a shared Top Event Mode lead to prevent serious accidents.
