Source Localization in Remote After Loading System Using CT Image and Radiograph

(in English) RALS (Remote After Loading System) which is a kind of radiation therapy are widely applied to the treatment for cervical cancer. In RALS, first a pipe called an applicator is inserted along the cavity within a tubular organ such as esoghagus and uterus from outside a body so that its head is positioned on the tumor region, and then a wire with a radiation source mounted on its tip is inserted through the applicator to irradiate the tumor region. If the wire tip is positioned away from the tumor region, the normal tissues are damaged by radiation. In this study, in order to avoid the risk, we considered a method to estimate the irradiation position using a preoperatively acquired computed tomography (CT) image for radiation treatment planning and an intraoperatively acquired radiograph. We registered the projection image generated from the CT image with the actual radiograph based on the position of Pb markers using a rigid body transformation to estimate the difference between planned source position and actual source position. We demonstrated the efficacy using simulated images generated form an actual clinical CT and actual images acquired from a physical phantom.
