近年,脳組織への非侵襲的かつ局所的な刺激に関心が高まっている.TMSはこの手法の一つであり,運動に関する脳機能のマッピングに利用されている.マッピングでは一般的に,TMSコイルにより発生する磁界が最も大きくなるコイル直下の位置が刺激した位置であるとして行われ,特定の反応を示した位置を記録して行われる.一方で,脳は複雑な構造を有しているため,刺激されている位置がコイル直下からずれることが考えられるため,刺激位置および特定反応を示すホットスポットを特定することは容易ではない.本稿では,実際のマッピングのデータを使用し,算出した電界分布からホットスポットの推定を行う.施術時と同条件で電磁界解析を行うため,被験者のMR画像からリアル人体頭部モデルを作成し,マッピング時のコイルの姿勢を用いて行った.最後に,推定したホットスポットを,術中の直接刺激の結果と比較した. |

Transcranial magnetic stimulation (TMS) is used for peritumoral mapping of the motor cortex. The typical procedure to estimate the stimulation site is projecting on the brain cortex the position where the maximum magnetic field is generated by the TMS coil. However, this approach is limited owing to the complex brain structures that can shift the hotspot position from the expected. Our work estimates the hotspot with higher precision than common procedure by combining the computed electric field strength distribution during multiple sessions of the peritumoral mapping. The electric field induced by TMS was computed using realistic human head models constructed from magnetic resonance images of patients and coil position/angle obtained by navigated TMS. Finally, the hotspot results were compared to the stimulation site localized via intraoperative direct brain stimulation. |
