Discussion on Stimulation of Direct Electrical Stimulation of Brain Subcortical Stimulation Based on Electromagnetic Analysis

(in English) In recent years, there has been a growing interest in awake surgery for brain tumor removal. During awake surgery, direct subcortical stimulation is applied at the resection cavity to guide the surgeon and avoid the risk of lesion of the pyramidal neurons in the cortical spinal tract in the white matter, so the tumor can be removed reducing the loss of motor or linguistic functions. The response to the stimulus is confirmed by electromyography recordings. In order to improve the discrimination of tissues in the vicinity of the brain tumor, the activation of the electrical stimulation needs to be characterized and quantified. Although monopolar and bipolar electrodes are used for electrical stimulation in the same operation, it is assumed that the stimulation region changes according to the type of electrode probe and its geometry. No studies have been conducted on the differences in stimulation areas due to the different characteristics of such electrode using biophysical simulation models. In this study, we computed voltage and electric field in the brain by scalar potential finite difference method in a realistic human head model made from MR image. And we obtained the action potential generated in a pyramidal neuron model to evaluate how the activation varies according to electrode conditions.
