Optimal Coil Orientation in TMS Targeting the Primary Motor Cortex

(in English) In recent years, there has been a growing interest in non-invasive stimulation of the brain for medical diagnosis. Transcranial magnetic stimulation (TMS) is a non-invasive scheme that induces an electric current below the stimulating coil, in which a pulse is injected through its windings. TMS has been used for brain mapping of language and motor function. When stimulating the primary motor cortex with a figure-8 coil, the optimal coil orientation is considered to be the direction in which the induced current flows roughly perpendicular to the gyrus of the primary motor cortex. However, because the shape of the brain differs from person to person, the angle rotated 45° from the direction from the back to the front of the head has been widely used as a reference orientation. There are few examples evaluating the variation in the induced electric field due to the orientation when the coil is placed perpendicular to the gyrus of the primary motor cortex, and the number of models used to study the optimal coil orientation has remained below 20. In this paper, we evaluated the angle dependence of the induced electric field in the primary motor cortex at the group level by placing coils directly above the primary motor cortex on a total of 135 realistic human head models and conducting electromagnetic field simulations by changing the coil orientation, and examined the optimal coil orientation.
