A proposal of Look Back Method for preventing a recurrence of the failure during system operation

(in English) When failures occur,
system developers cope with the failures, and analyze
the causes.They scrutinize recurrence preventions and apply them to their future projects.
However, considerable similar failures continue to occur.
In this paper,
we propose a method called the Look Back Method in order to prevent
recurring failures.
We show results from causes for past failures and measures to follow for recurrence prevention in enterprises.
We have applied the Look Back Method to a project in an enterprise
and evaluated the results.
As results go,we have detected failure-prones which could not be detected by the utilizing conventional methods e.g. the checklist method,and thus, have validated the Look Back Method.
