嚥下障害者の嚥下能力評価 ～ 表面筋電図と嚥下造影図による検討 ～

近年，高齢化や疾病の重症化に伴い，嚥下運動に障害のある患者が増加しており，これは誤嚥性肺炎の原因となるため問題となっている．そこで我々は，嚥下機能の客観的評価法として，非侵襲的な方法である表面筋電図を計測し，同時記録した嚥下造影図を参考とすることで，嚥下障害者における嚥下運動時の表面筋電図の特徴を検討した．被験者は，高齢健常者群として男性1名，女性4名（65～75歳），嚥下障害者群として男性6名，女性2名（57～83歳）とした．被験者にはＸ線撮影室内で角度30 度のベッドに安静仰臥位を保ってもらい，検者から投与された造影剤含有ゼリー2.5mlを任意に咀嚼，嚥下してもらい，そのときの表面筋電図と嚥下造影図を同時に記録した．その結果，嚥下障害者は健常者と比較して，筋活動持続時間および咽頭期筋活動開始時間差が増大し，オトガイ舌骨筋の咽頭期単位筋活動量と，咽頭期単位筋活動量比が減少することがわかった．また，各嚥下障害者のパラメータの特徴を検討した結果，嚥下障害の程度や障害の原因と，表面筋電図に表れる特徴との関連性がみられた．以上の結果より，表面筋電図が嚥下機能の客観的評価方法として有効である可能性が示唆された． |

Recently, the relationship between pneumonia and aspiration is designated. It is deliberated that the number of the aspiration patients is on the increase because of increasing of old people or patients with serious movement disorders. In this research, differences of characteristics of deglutition between healthy subjects and dysphagia patients were investigated using surface electromyogram (sEMG) as a method for evaluation of swallowing with the videofluorography (VF) confirmation. Five healthy elder (65-75 years old, one male and four females), and eight dysphagia patients (59-83 years old, six males and two females) were participated. Subjects were instructed to lie on a bed with 30 degree inclination and to swallow 2.5 ml of barium-coated jelly. During swallowing, sEMG signals were measured. As results, duration of muscle activity, muscle activity power per second, ratio of activity power per second between geniohyoid muscle and stemothyroid muscle in pharyngeal stage, and beginning time difference between geniohyoid muscle and sternothyroid muscle was different in the dysphagia group compared with the healthy group. Additionally, the relationship between a characteristic of dysphagia and that of sEMG might be available to associate. Therefore, it was suggested that sEMG could be effective for the objective evaluation of swallowing. |
