Lung tumor tracking based on thoracic 4D-MR image for radiation therapy

(in English) In radiotherapy in thoracic region, it is critically important to grasp the tumor motion due to respiration in order to avoid unnecessary exposure to healthy organ. In general, we track the tumor position with X-ray fluoroscopy. However, the tumor is sometimes hidden behind other organs and it makes the direct tracking very difficult. Clearly observed other organ can be used as a surrogate to estimate the tumor location in such case. In this paper, we propose an estimation scheme of tumor location with preoperative 4D-MRI. In this scheme, we first model the relationship between the tumor location and feature point location on diaphragm based on thoracic 4D-MRI. During treatment, tumor location is estimated from the diaphragm in the X-ray fluoroscopy. We estimated the accuracy of the proposed scheme through 4D-MRI of two healthy volunteers. As a result, we could predict the tumor location with accuracy of 0.38 [mm] on average if it is stable breathing.
