Analysis of “Hiyari-Hatto” Incidents Reported by Maintenance Service Engineers

(in English) The purpose of the research is to reduce “Hiyari-Hatto” incidents of hardware maintenance service engineers (hereinafter engineers) who participate in maintaining enterprise information systems. According to the frequency data of potential accidents which cause injuries explained in Herbert W. Heinrich's "Industrial Accident Prevention" law, for every accident that causes a major injury, there are twenty nine accidents that cause minor injuries and three hundred accidents that cause no injuries within three hundred thirty similar accidents which are caused by a single person. Hence classifying and analyzing the “Hiyari-Hatto” incident reports experienced by the engineers may help to reduce failures of enterprise information systems by reducing “Hiyari-Hatto” incidents. This article clarifies the characteristics of error classifications (slips, lapses and mistakes) using James T. Reason's theory and human performance levels (skill-, rule-, and knowledge-based) using Jens Rasmussen's theory by comparing them per product group. Herewith, promoting awareness and training methods can be realized to reduce “Hiyari-Hatto” incidents.
