The Transcranial Doppler method (TCD) enables the measurement of cerebral blood flow velocity and detection of emboli by applying the ultrasound probe to the temporal bone window, orbital, or greater occipital foramen. However, measurements often become difficult in elderly women. Among various factors for this problem, we focused on the effect of cancellous structure in the diploe on the ultrasound penetration into the brain. Using a 2D digital bone model of the temporal bone in the horizontal and vertical directions, wave propagation through the skull bone was investigated by the Finite-Difference Time-Domain (FDTD) method. We fabricated digital bone models with similar structure but different BV/TV (Bone Volume/Total Volume) in diploe. Around BV/TV of 50%-60% (similar to the BV/TV of elderly women), the observed amplitude of ultrasound in the brain showed minimum values due to multiple reflection and scattering in the cancellous bone of diploe. The results suggest that structural changes such as osteoporosis may be one reason for the difficulty of TCD measurements. |
