AI models for whole-body bone metastasis segmentation from CT images play a critical role in diagnostic support. However, model evaluation using conventional metrics such as the F1-score fails to adequately assess clinically important aspects regarding lesion geometry and detection performance. In this study, to measure the clinical utility of models, we introduce a comprehensive evaluation method that combines shape accuracy (ASSD, 95%HD), volume accuracy (RVD), qualitative error maps, and stratified analysis by lesion type and anatomical site. We applied this comprehensive evaluation method to evaluate the performance of a high-precision model (F1-score 0.931) constructed by the authors. As a result, it was suggested that the comprehensive evaluation method can clearly demonstrate the clinical risks associated with the model’s output, which are often obscured by a single F1-score evaluation. |
